The insurance companies need to be given a new job description that adds value to the product. Let's pay them to collect and evaluate clinical performance data for a new type of accountable health plans.
The drug companies need to be allowed to operate in the free market too a greater extent; a little competition on price won't hurt them.
Unfortunately, our limited imagination does not allow us to consider the most efficient model, a single payer system with centralized authority, well defined accountability, bargaining muscle, standard benefits and controls on manpower and capital investment. Let's look to the single payer systems we now have (Medicare and the VA) to see what works and what doesn't.
